The carnival ride at the Ohio State Fair that malfunctioned on Wednesday, killing one person and injuring 7 was inspected just hours before the horrific incident.
According to TIME, inspectors repeatedly looked over the “Fire Ball” and signed off on it just hours before an entire row of the gondola wheel broke apart, ejecting at least two people into the air.
Tyler Jarrell, 18 was thrown about 50 feet and was pronounced dead not far from the ride. He had just graduated high school and was enlisted in the Marine Corps. His girlfriend was among the seriously injured. At least two are listed in critical condition, according to TIME.
